Membership Form
I wish to become a member / renew my membership of Suffolk Oxygen Therapy Centre for one year. I understand that membership runs from 1st January to 31st December and agree to pay the sum of £50, per person, per year.
In the event of the company being wound up, I undertake to pay a sum not exceeding £10 towards any outstanding debts in accordance with companies’ regulations.
The Suffolk Oxygen Therapy Centre (SOTC), also trading as Suffolk Multiple Sclerosis Therapy Centre (SMSTC), is a self-funded charity helping people with Multiple Sclerosis, as well as a number of other conditions.
